The Challenge
A care home rated 'Requires Improvement' by CQC in 'safe' due to poor PRN documentation
A care home experienced two medication errors in quick succession where controlled drug (CD) patches were left on a resident, increasing the risk of overdose. There was no consistent documentation or checking process in place for patch application or removal.
A care home was experiencing frequent issues with its medication ordering process - items were arriving out of sync, some medications were delivered that hadn’t been ordered, and waste was increasing. The inconsistent supply was also contributing to medication errors due to stock shortages.
What We Did
We delivered targeted staff training focused on writing clear, patient-centred PRN protocols. The session included real examples, practical discussion, and a new PRN protocol template designed to meet CQC and NICE standards.
We introduced a new CD patch application record, including:

A daily patch check log

A dedicated section to sign for patch removal

Clear space for both applier and witness signatures
We also provided staff training on the risks of multiple patches and the importance of safe CD handling.
We supported the home to set up online ordering, working collaboratively with their GP surgery and pharmacy to agree a 28-day ordering protocol. This included:

A clear process for new residents

Guidance on how to request interim prescriptions without disrupting the cycle
The Outcome
At reinspection, the service was rated ‘Good’ in Safe. Staff reported increased confidence, and safeguarding referrals related to medication significantly reduced.
Since implementing the new process over a year ago, no safeguarding referrals have been made for patch-related issues. Staff now take ownership of safe patch administration, and witnesses understand their shared responsibility in the process.
The home now only receives the medications it needs, on time and in sync. Staff understand how to keep new residents aligned with the ordering cycle. The result has been a significant reduction in medicine waste and fewer medication errors due to stock issues.
